This is particularly true when you need physical therapy, intravenous services, or wound care. And if you only need IV services once a day or simple wound care, you may be able to receive those services at home. Most seniors who need help paying for an assisted living facility get a long-term care insurance policy. Alternatively, if you qualify for Medicaid, you may be able to get some help paying for assisted living care.
Choosing a Nursing Home
Depending on the member benefit, Kaiser Permanente does cover intermittent therapy needs identified in the LTC/Custodial Care setting. The therapist in the nursing home will submit orders and therapy evaluations to Nursing Home Services in order to get authorization for this service. While other options are available to you for skilled nursing care, only our premier SNFs can offer this high level of coordination. You can rest easier knowing that our SNFs and your health care team have a shared commitment to your health and well-being and are working together for the best possible outcome.
Availability varies by location and need, but telehealth allows you to talk directly to a nurse or doctor without leaving the comfort and security of your home. You can use this service to refill subscriptions, ask questions, get medical advice, and schedule future appointments. Patients stay in SNFs only until they have recovered enough to move back home or to another living arrangement. Your Kaiser Permanente team will decide if you need care in a skilled nursing facility.

They are conveniently located to make it easier for friends and family to visit during your stay. Skilled Nursing Facilities and Acute Rehabilitation Facilities are responsible for Admission Notification for inpatient services. For weekend and Federal holiday admissions, notification must be received by 5 p.m.
Most training programs are several weeks to months in length. They provide training to care for elderly, homebound, and ill people. A licensed practical nurse or registered nurse should be available at all times to oversee the care given by the CNAs. Are hospice or other specialized services (such as Alzheimer's care) offered? If they're available, is there a waiting list for these services?
Types of Care
Our care management nurses work closely with a facility's staff to conduct reviews and coordinate care. If the admission or continued stay appears to be inappropriate, our nurses, in conjunction with the patient and the multidisciplinary team, will develop an alternative plan. Once a member selects a nursing home for long term care, their primary care provider or personal physician will work with clinic social worker to request placement. Please contact the nursing home directly to determine all paperwork required.
If the need is established following admission, Contractor shall contact the CMLN. Not all treatments or services described are covered benefits for Kaiser Permanente members or offered as services by Kaiser Permanente. For a list of covered benefits, please refer to your Evidence of Coverage or Summary Plan Description. For recommended treatments, please consult with your health care provider.
Our SNFs provide physical, occupational, or other rehabilitative or medical therapies. We emphasize quality and timely rehabilitation with coordinated care. Some people have long-term care insurance that helps cover some costs. HCFA provides Medicare and Medicaid reimbursement to nursing homes to provide care for the elderly and disabled. HCFA regularly inspects nursing homes to ensure that Medicare and Medicaid guidelines are met. Do not hesitate to ask the facility about its most recent state and federal inspections.

You don’t have to worry about collecting your medical records and test results. We already have them and your Kaiser Permanente SNF health care team can access them anytime. Our goal — just like yours — is to get you better and back in the comfort of your home as soon as possible. Because the care is so well coordinated, we’ve found that members who stay in our SNFs are often able to go home sooner than those who stay in other facilities.

When you qualify for Home Health services, our home health aides can help with short term intermittent personal care. Our social workers can help you identify and secure Kaiser Permanente and community resources to improve your overall health. They can also provide counseling to help you deal with the impact of illness or long-term care.
If you don’t live in one of the aforementioned states, you may need to look elsewhere for a private Medicare provider. Alternatively, if you do live in one of these states or areas, you may find an affordable Medicare plan to fit your needs. Contractor shall submit invoices for these items or services in accord with this Agreement. Payment by Health Carrier for these excluded items or services will be at Contractor’s cost, which cost must be supported by original invoices. Authorization for items or services not included in the per diem must be obtained in advance from Nursing Home Services SNF Placement Office prior to admission.
